This form is only to be used for enquiries and information requests to the Isle of Wight LADO Service and we encourage you to make contact via this enquiry form.
However should you wish to complete a full LADO referral form in the first instance. Please call the LADO service on (01983) 823435 or email LADO@IOW.gov.uk and a referral form will be sent to you.
If you think a child is at immediate risk of significant harm please call the Police and or Isle of Wight MASH immediately.

Notifications and or referrals are made to the LADO if it is believed that an adult working with children has:
Behaved in a way that has harmed or may have harmed a child
Possibly committed a criminal offence against, or related to a child
Behaved towards a child or children in a way that indicated they may pose a risk of harm to children
Behaved or may have behaved in a way that indicated they may not be suitable to work with children
The information provided in this enquiry form will help us determine if a LADO referral and investigation is required and you will receive a response to this enquiry within one working day.
